1. Acceptance of Terms By accessing and using AccelerantMedical, PLLC ("we" or "our") website, you agree to be bound by these Terms and Conditions. If you do not agree, please do not use the our website.

2. Services Provided We provide concierge medicine, family medicine, and telehealth services, and medical AI assistance for billing, transcription, and after-visit summaries. These services do not replace in-person medical care when necessary.

3. User Responsibilities

• You must be at least 18 years old or have parental consent to use our services.
• You agree to provide accurate information and comply with all applicable laws.
• You are responsible for maintaining the confidentiality of your account credentials.

4. Medical Disclaimer We do not provide emergency medical services. If you are experiencing a medical emergency, call 911 immediately. The information provided on our website is for informational purposes only and should not be construed as medical advice.

5. HIPAA Compliance We are committed to safeguarding your protected health information in accordance with HIPAA regulations. Protected Health Information (PHI) is collected and stored in the patient portal, and in accordance with the Push Health® Privacy Policy. Therefore, in order to continue to safeguard your information, when requesting service through our website, please login to the patient portal.

6. Limitation of Liability We are not liable for any damages resulting from the use of our website or services. We do not guarantee the accuracy of AI-generated medical billing codes, transcription, or treatment recommendations.

7. Termination of Use We reserve the right to suspend or terminate your access if you violate these Terms.

8. Changes to Terms We may update these Terms at any time. Continued use of our website constitutes acceptance of any modifications.

9. Governing Law These Terms shall be governed by the laws of Utah, without regard to conflicts of law principles.

By using the patient portal, you consent to be redirected outside of our website to Push Health®, which is the Electronic Medical Record (EMR) and telemedicine platform used by our website to provide Telehealth medical services through a Business Associate Agreement (BAA). Push Health® is not affiliated with our website, and is independently maintained, owned, and operated.
